C4P 16-18 yr old registration
This registration event is for players aged 16-18 who are taking part in C4P. To register, you must:1. Download and fill out the assumption of risk document
. You can either print it out and fill it out with ink, or fill it out electronically with an electronic signature (see apps below).
2. Upload it electronically on this page. You can submit it as a photo or a PDF.
